EIHAB Human Services Program Manager in Springfield Gardens, New York

Salary: $58,500 60,000 Essential Responsibilities: Coordinates delivery of services with Day Habilitation Program Services; parents of those living at home. Assist in the developmental of individual team productivity, allocates resources and staff. Monitor and supervise Day Habilitation Client Coordinators and Direct Care Worker. Conduct staff meeting as needed with Field Supervisor, Direct Care staff and Client Coordinators. Review all paperwork prepared by the Client Coordinators on a regular basis. Responsible for evaluating the staffs performance and writing and communicating performance related issues. Conduct monthly staff meeting with Day Habilitation Staff. Schedule fire drills and emergency evacuation drills for all Day Habilitation participants, chart their progress and maintain records. Assist in the consumers advocate in obtaining all services outlined in the Day Habilitation plan. Responsible for completing monthly progress notes for all participating consumers. Responsible for keeping the financial record and following up of the facility. Ensure that all Day Habilitations document for each consumer is updated. Complete reports and ensure proper documentation of any incident at the facility. Ensure that the Client Coordinators are maintain all consumers Day plans and update each plan as needed. Ensure that the Client Coordinators are developing goals for the consumers, monitors consumers progress and review such progress on a daily basis, documents all outcomes and make appropriate recommendations. Ensure the Client Coordinators are attending all individualized service plan meetings, and applicable review. Supervise the maintenance of assigned staff records regarding PPDs and training certification. Conduct staff evaluation Coordinate with Program Director to identify staff needs, staff training etc. Ensure completion and proper documentation of fire drills. Train new hires and provide an orientation, to ensure that the staff is clear about their responsibilities. Ensure the Day Habilitation program is adequately staffed, based on regulatory ratios. Provide supervision and guidance to the Field Supervisor. Performs all other duties assigned. Qualifications: Bachelors degree in the Human Services related field, with one year of related work experience, Associated degree, with (5) years of related work experience or a High School Diploma/GED, with 10 years of work experience with the developmentally disabled community. SCIP, AMAP, and CPR/First Aid certifications preferred and Familiarity with OPWDD regulations. Employee must have a valid New York State Drivers License. Required Knowledge, Skills and Abilities: Employee must possess the ability to interact with individual and staff professionally and ethically, understand and implement service plans, and to complete all required documentation clearly, legibly in a timely manner.
