Covenant Health PATIENT COMPANION in Nashua, New Hampshire

Summary

The Patient Companion Provides observation to patients within an assigned department of the hospital.

Essential Duties and Responsibilities

  • Supports and promotes the mission and values of Covenant Health Ministry.

  • Provides observation to patients within an assigned department of the hospital.

  • Follows policies and procedures outlined by the Nursing Division and SJH.

    • Observes the safety of the patient.
    • Orients and reorients the patient as appropriate
  • Identifies and reports potential alterations in safety, including environmental hazards.

  • Anticipates need for assistance in providing safety for the patient.

  • Remains in the room of the patient, requiring constant observation, until relieved by another staff member.

    • Obtains shift report and plan of care for shift from supervising Registered Nurse.
  • Verbalizes appropriate use of assistive devices such as hearing aids, glasses, teeth, etc., and can assist patient and/or family in placing these items on the patient.

  • Call nursing personnel to attend to any patient care needs.

  • Reports behavior and/or cognitive changes to nursing personnel.

  • Upon completion of appropriate training and competencies will assume the role of continuous visual telemonitor (CVT) as needed.

  • Maintains cleanliness of patient care areas

  • Maintains annual competencies

  • Attends all mandatory training as assigned within the timeframe given

  • Other duties as consistent with this role.

    Job Requirements

    Job Knowledge and Skills

  • Crisis Prevention Training (CPI) preferred or obtained prior to completion of orientation.

    Education and Experience

  • High School Diploma or GED preferred

  • Healthcare experience preferred

  • Basic Cardiac Life Support preferred

  • Geriatric or Mental Health experience preferred

    An equivalent combination of education and experience which provides proficiency in the areas of responsibility listed above may be substituted for the above education and experience requirements.

    Working Conditions/Physical Demands

    Must possess the physical and mental abilities to perform the tasks normally associated with this position that involves a combination of sitting, standing, walking, bending, stooping, and reaching. Lifting/carrying frequently up to 35 pounds, and occasionally up to 50 pounds. Some stress related to high level of responsibility for quality care.
