The Research Foundation for The State University of New York at Stony Brook Registered Nurse - Education/Clinical Quality Outcomes in Commack, New York

Required Qualifications (as evidenced by an attached resume):

A Diploma from an accredited school of Nursing or a Bachelor's degree (foreign equivalent or higher) in nursing or related medical field. Two (2) years' medical experience in a clinical, hospital or ambulatory setting including medical quality assurance experience. Current New York State Registered Nurse. Strong knowledge of an Electronic Medical Records system. Experience with reporting descriptive data utilizing Microsoft suite or equivalent software.

Preferred Qualifications:

Master's Degree (foreign equivalent or higher) in Nursing or Medical field, or at least four (4) years of full-time nursing experience. Experience with medical service compliance, case management, auditing medical records and providing data reports to management. Experience with the following patient populations: PTSD/depression, cancer, lung disease and sleep apnea. Experience working with government funded programs. Familiar with 9/11 Responders or the first responder population.

Brief Description of Duties:

The RN's role will be to ensure high level of patient care and compliance through medical education and outreach. As a point of contact for patient outreach, the RN will educate patients on cancer screening and important testing to motivate adherence to the diagnostic and treatment plans. The RN will monitor and measure clinical quality outcomes through chart reviews and audits. The RN will work closely with RN case managers to enhance continuity of care, ensure that there is cohesion and collaboration between the two clinic specialties between clinical visits. The RN will provide support to the LPNs and conduct annual monitoring questionnaires, collect research data and provide education to patients seen in the clinic for monitoring and treatment. The RN will also perform phlebotomy, spirometry testing, and cognitive assessments.

The ideal candidate will thrive in a team environment, show a collaborative spirit, be an excellent communicator, and must be able to navigate through multiple clinical paths simultaneously.

  • Collect medical data during annual monitoring exams and perform phlebotomy, spirometry and cognitive tests.

  • Review and communicate urgent or stat results to the provider to follow up with member. If directed, follow-up with member providing required medical information and education.

  • Be a point person for incoming calls from member services line or messages in patient portal to deal with medical questions from our members. Triage calls and deal efficiently with member questions.

  • Review lab reports, ColoGuard reports, radiology reports, cancer screening results and other important test results, appropriately triage, and ensure timely follow-up communication with the members.

  • Play a key role in member adherence to appointments with community providers, by identifying non-compliant members and outreaching to these members to motivate timely follow-up. Work collaboratively with Case Management and Member Services to enhance our compliance program and case management program.

  • Ensure Quality Assurance in the Clinical areas: review records, measure outcomes, provide reports, implement and assess improvements. Provide training and feedback to staff to ensure compliance, safety and quality.

  • Participate in quality assurance committee meetings to ensure collaborative solutions and help the teams meet performance and quality metrics and goals. Participate in quality assurance projects, audits, chart reviews and other data collection projects. Implement clinical improvement projects under the direction of the Nursing Supervisor in order to meet Program QA goals.

  • Other duties as assigned.

FLSA Exempt position, not eligible for the overtime provisions of the FLSA. Minimum salary threshold must be met to maintain FLSA exemption.
